Home care for older Australians is the setting for this Sydney job. The provider offers person-centred care through the Support at Home program and wants a compassionate, experienced Care Partner / Case Manager to join a close-knit team that serves clients across the city.
You would lead on care coordination and individual support plans, meeting clients face to face. The team has a flat structure, so you work autonomously and make sure each person gets the right services at the right time. Visa sponsorship is also available for the right candidates.

Key Responsibilities
- Keep good working relationships going with clients and their families, as well as support workers and other providers
- Arrange services that suit each client's own goals and health needs
- Assess each client, then prepare and review their care plan
- Weigh decisions by what keeps clients safe, well and living a good life
- Supervise and lead the Support Workers so service stays consistent and good
- Watch and manage the budget for each client
- Follow the law, funding rules and the organisation's policies
- Write progress notes promptly and accurately, read notes from support workers, suppliers and allied health staff, and welcome new clients with intake assessments and meet and greets
Requirements & Eligibility
- A track record in aged care, home care, community services or health settings
- Good at assessing clients, planning care and managing cases
- Self-directed, able to balance competing priorities
- Communicates well, builds relationships and leads people effectively
- Practical knowledge of My Aged Care and the Aged Care Quality Standards
- Has a driver's licence and their own vehicle, and is willing to travel across Sydney
- Being bilingual or multilingual is a strong advantage; a clinical background in nursing or allied health is beneficial but not required
